To request a copy of your medical record(s), you must complete an Authorization to Request Medical Records. The request may take 10-15 days to process.

Request a Medical Record Amendment
To request an amendment to your medical record(s), you must complete a Request for Amendment of Protected Health Information Form. The request may take up to 60 days to process.
Please email the request along with your photo identification and a copy of the information you would like to be reviewed to the facility where you're requesting the change be made at:
