Welcome to the Patient Portal!

Our patient portal offers patients of Baptist Health Facilities a simple and convenient way to manage their healthcare, how, where and when it’s best for them. Your account is completely secure, FREE to use and is available 24 hours per day.

This portal offers simple navigation that allows you to securely access your personal information at the click of a mouse. To request access to your patient portal, simply complete the Patient Portal Access Request Form. This request form must be sent to patientportalsupport@BaptistFirst.org from a valid email address (cloud messages are not accepted) or returned in person to the facility's Health Information Department from which you are requesting your records. Please allow 7 to 10 days to process your request.

Please note that your entire medical record is not available through this portal.

THROUGH YOUR PATIENT PORTAL ACCOUNT YOU CAN:

  • Obtain health education information in your Continuity of Care document.
  • Easily view your lab results, with many available within 72 hours.
  • View discharge instructions.
  • Manage the health of your children (under 14 years of age) or a loved one to whom you have been designated to serve as a health care proxy. 
    *Please note that the medical information for minors, between the ages of 14-18, will not be available here. If you are requesting access to the portal on behalf of a patient (as a health care proxy), please submit the Patient Portal Proxy Access Form, with all requested supplemental documentation to patientportalsupport@baptistfirst.org.
  • View your medication list.
  • View your medication allergy list and medical history.
  • Send messages to your Baptist Health Primary Care Provider.
  • Access your account through any desktop or mobile device.
  • View your upcoming appointments.
  • Please send a completed form with a copy of your government-issued ID or driver’s license to patientportalsupport@baptistfirst.org.

Once you receive your log-in email, you have 90 Days to enroll before your log-in expires
