Through Your Patient Portal Account, You Can:
- Obtain health education information in your Continuity of Care document.
- Easily view your lab results.
- View discharge instructions.
- Manage the health of your children (under 14 years of age) or a loved one to whom you have been designated to serve as a health care proxy.
- Please note that the medical information for minors, between the ages of 14-18, will not be available here.
- If you are requesting access to the portal on behalf of a patient (as a health care proxy), please submit the Patient Portal Proxy Access Form, with all requested supplemental documentation and a copy of your government-issued ID or driver's license to patientportalsupport@baptistfirst.org.
