Please print and complete the forms appropriate to your exam and bring with you to your appointment, or forms may be faxed to the NeuroScience Imaging Center at (334) 286-3165, before your arrival.

Please bring a current list of your medications to your appointment.

Valet parking

Patients visiting the NeuroScience Imaging Center may use the valet service at the Parker Pavilion free of charge (the valet stand is located at the front of the Parker Pavilion).
